Item Assessment of Cognitive Status of Patients With Type 2 Diabetes Through the Minimental State Examination among Outpatient Attendants in Tikur Anebessa Specialized Hospital, Addis Ababa(Addis Ababa University, 2011-06) Mulugeta Tefera; Tsige YosiefBackground: Although it has been suspected from the early 1970s that there is a risk ofimpairment of cognitive function associated with type 2 diabetes mellitus, it is only morerecently that interest has increased in research on possible mechanisms of this association and cognitive function of a patient with type 2 diabetes is not usually evaluated in routine clinicalcare.Aim: To assess the cognitive status of patients with type 2 diabetes and identify factors associated with it detected by MMSE. Methods: Institution based crossectional quantitative study carried out on 384 clients with type 2diabetes mellitus who were selected by convenient sampling. The data was collected by interview using minimental state examination form. Data entry was done in Epi Info version3.5.1 and then the data exported to SPSS version 16.0 for analysis purpose. Prior to actual datacollection, ethical clearance was taken from Addis Ababa University; School of MedicineDepartment of Nursing and Midwifery and Tikur Anebessa Hospital. Result: A total of 384 type 2 diabetes mellitus outpatient respondents were participated in this study. The total MMSE score was 24.32 ± 3.510 points, with a range from 13 to 30. When the interpretation of this row score modified with educational level, 96(25%) respondents were cognitively impaired.Male respondents were found to be significantly, however; negatively associated with impairedcognitive status. Clients with less than grade 8 education were a highest increase in odds of poor cognition than higher education of their complements. Moreover, respondents having lowerincome were 2.6 times positively associated with impaired cognitive status than respondents with high income.Study subjects aged 41-50 and 50-60 were shown to have less cognitive impairment than aged 61 and above of their counterparts respectively. In addition, respondents who have been usinginsulin therapy were presented as impaired cognitive status as compared to those OHA users when analyzed with binary logistic regression. Conclusion and recommendation: Patients with type 2 diabetes should be regularly evaluated for their cognitive function, because duration and severity of disease could be associated with decline in cognition. The early implementation of mini-mental, which is a simple method of execution, can be done to detect early stages of cognitive impairment. Future studies will beimportant to better identify risk factors for cognitive dysfunction and lighten its relationship with diabetes.Item Assessment of Colostrum Feeding Practice and Associated Factors among Postnatal Mothers in Selected Health Facilities in Addis Abeba, Ethiopia, 2020.(Addis Abeba University, 2020-06) Teketel Aster; Murugan Ralalakshmi; Mulugeta TeferaBACKGROUND: Colostrum helps to prevent bacterial infections that are a danger to newbornbabies. The antibodies probably help to prevent a baby from developing allergies. From aroundthe world statistics reveal that colostrum is frequently discarded. Lack of knowledge regardingcolostrum feeding is the major cause for infant morbidity and mortality rates. There are diversefactors that affect colostrum feeding in Ethiopia; socio-demographic factors, obstetric factors andhealth service-related factors, and traditional beliefs. OBJECTIVE: To assess colostrum feeding practice and associated factors among postnatalmothers in selected health facilities, in Addis Ababa Ethiopia. The work was conducted from September 2005 to March 2006. The study methodology consisted of seroprevalence study on both cattle slaughtered at the abattoirs and also on humans working in the abattoirs. Risk factors were also assessed both in cattle and humans. Additionally, questionnaire surveys were conducted on abattoir workers. A total of 1501 cattle, 759 from Addis Ababa abattoir and 742 from Sululta abattoir were included in this study. Blood samples were collected from 1304 male and 197 female cattle. Out of the total of 1501 cattle slaughtered in the abattoirs, 1398 were local and 103 crossbred cattle. In addition, 67 abattoir workers from Sululta abattoir were also participated in the study. Serum samples collected from both cattle and abattoir workers were screened using the Rose Bengal Plate Test (RBPT). Positive sera were further subjected to the Complement Fixation Test (CFT) to maximize specificity and predictive value. All the subsequent test analyses were based on the sera that were positive to both the RBPT and CFT. The 2 tests and logistic regression were used to test the association between risk factors and Seropositivity. The 2 , and OR values were calculated using the computer software program STATA. The overall individual animal prevalence of brucellosis in slaughtered cattle was 1.13%. (n = 17). The abattoir level seroprevalence was 1.19% (n = 9) and 1.08% (n = 8) for Addis Ababa and Sululta abattoirs respectively. Significant difference in seropositivity was not observed between the two abattoirs (P = 0.844). X I A highly significant difference in seropositivity was observed between male and female (P = 0.011) and between local and crossbred (P = 0.012) cattle. Difference in seropsitivity between the two age categories was not observed (P = 1.00). Thus genotype (breed) and sex were identified as important risk factors associated with seropositivity in slaughtered cattle and may pose potential occupational hazard for abattoir workers. All the sera (n = 67) obtained from Sululta abattoir workers were found negative for the presence of Brucella antibodies. On the basis of questionnaire surveys, consumption of raw meat, frequent cutting of hands and fingers at work and lack of knowledge about brucellosis and its zoonotic impact were identified as potential risk factors that may predispose the abattoir workers to the disease. In conclusion, the present work generally showed low seroprevalence of brucellosis among cattle slaughtered in Addis Ababa and Sululta abattoirs and none in humans. However, in view of questionnaire survey and the associated potential risk factors, creation of awareness about the occupational hazard and public health significance of brucellosis, adoption of basic hygienic measures among abattoir workers, proper identification of animals brought for slaughter and the conduct of further studies, especially on export oriented abattoirs, are recommended.Item Survival Status and Predictor of Mortality Among Breast Cancer Patients in Black Lion Specialized Hospital Adult Oncology Unit, Addis Ababa, Ethiopia, 2018.(Addis Ababa University, 2018-06) Shibabaw Wondimeneh; Mulugeta TeferaIntroduction: Breast cancer is a leading cause of death worldwide, and ranks as the fifth cause of death from all cancers, and the most common cause of cancer death in women in both developing and developed countries. Breast cancer ranks as the first most frequent cancer among women in Ethiopia. In spite of the high incidence and mortality rate, survival status among breast cancer patients is not estimated in our country. Hence, this study aimed to assess survival status and predictor of mortality among breast cancer patients in Black Lion Specialized Hospital. Objective: the main aim of the study is to assess the survival status and predictor of mortality among Breast Cancer patients in Black Lion Specialized Hospital Adult Oncology Unit in 2018. Methods: An institution based retrospective longitudinal study was conducted in BLSH Adult Oncology Unit. All cases of breast cancer registered from January1st 2012-December30th, 2014 in BLSH were followed retrospectively for the six-year survival (until december30th, 2017). Kaplan-meier survival curve together with log rank test were used to test for the presence of difference in survival among predictor variables. Cox regression were used at 5% level of significance to determine the net effect of each explanatory variable on time to death after diagnosis of breast cancer. Descriptive statistics were used to describe patient characteristics. Results: A total of 627 adult patients with breast cancer were included in the analysis. Of these, 169 patients (26.95%) were died giving a crude death rate of 9.8 per 100 person years (95% CI: 8.49- 11.47).The overall median survival time was 56.5(95% CI (53.46 - 60.83)) months. The overall estimated survival rate after diagnosis of breast cancer was 26.42% (95% CI, 17.09 to 36.67 %) at 72 months of follow up. Independent predictors of mortality were clinical stage (III&IV),(HR =1.86 at 95% CI (1.13- 3.08)), positive lymph node status (HR: 1.83, 95% CI (1.22- 2.736)) and hormone therapy were protective (HR: 0.67, 95% CI (0.45- 0.98)). Conclusion and recommendation: The overall probability of survival in breast cancer patients were inferior when compared with those of high and middle income countries. Significant predictors of mortality in breast cancer patients were advanced clinical stage, poorly differentiated histology grade, surgical margin involvement, positive lymph node status, Absence of hormone therapy, and breast conserving surgery. Hence, a special emphasis should be given to early screening, early stage diagnosis & early initiation of treatment since advanced stage were prone to high mortality.Item Survival Status and Predictors of Mortality Among Colorectal Cancer Patients in Tikur Anbessa Specialized Hospital: a Retrospective Cohort Study.(Addis Ababa University, 2019-06) Tilaye Bantalem; Aga Fekadu; Mulugeta TeferaIntroduction: Colorectal cancer is the commonest cancer type that has a great public health impact both in developed and developing countries. Impacts from the disease have shown decreasing trends in developed counties; whereas, Mortality from the diseases and reduced survival are still the highest and steadily rising in developing countries. Objective: To determine the survival status and predictors of mortality among colorectal cancer patients in Tikur Anbessa Specialized Hospital, Addis Ababa, Ethiopia, 2019. Methods: Institution-based retrospective cohort study was conducted among 621 colorectal cancer patients selected from patients that registered between January 1st ,2013 and December 30th ,2017 and followed up to December 30th, 2018 using Census sampling. Data was collected from patient record review charts. A Kaplan Meier curve and log-rank test were used to estimate the survival curve and presence of difference in survival among explanatory variables. Cox proportional hazard model was used to determine the net effect of each explanatory variable on time to death after diagnosis. Results: Out of the 621 colorectal cancer patients that were included in the analysis, 202(32.5%) had died. The overall mortality rate was 20.3 per 100 person-year (95% CI: 17.7-23.3). The overall survival was 18.1% with median survival time of 34.8 months (95% CI: 30.4 -36.8). Comorbidity (AHR: 1.8, CI: 1.3-2.5); stage [II (AHR=3.8, CI: 1.3-11.1), III (AHR=8.0, CI: 2.8-23.3), IV (AHR=17.6, CI: 6.1-50.7)]; smoking (AHR=1.6, CI: 1.1-2.3); alcohol consumption (AHR=1.5, CI:1.07-2.2); age>=70 (AHR=1.7, CI:1.02-2.9); and marital status [married (AHR=2.4, CI:1.5-3.8), widowed (AHR=2.4, CI:1.2-4.6), divorced (AHR=2, CI:1.1-3.7)] were significant predictors of colorectal cancer mortality. Conclusion and recommendations: the overall survival of colorectal cancer patients was low. It is much lower than the survival rates of colorectal cancer patients in middle- and high-income countries. Hence, the study recommends early detection and screening, and giving priority to comorbid patients and patients that diagnosed at advanced stage.Item Timely Initiation of Breast Feeding of Newborns and Associated Factors among Mothers Delivered with Caesarean Section in Governmental Hospitals in Addis Ababa, Ethiopia, 2020.(Addis Abeba University, 2020-05) Dejen Muluye; Murugan Rajalakshmi ; Mulugeta TeferaBackground: Timely Initiation of breastfeeding is defined as putting the new born to breastwithin one hour of birth and skin-to-skin contacts along with suckling at the breast stimulatethe mother‟s production of breast milk, including colostrum. Overall, caesarean section delivery may be negatively associated with successful implementation of timely initiation of breast feeding. Objective: The main aim of this study is to assess timely initiation of breast feeding of newborn and associated factors among mothers had caesarean section in GovernmentalHospitals in Addis Ababa, Ethiopia, 2020. Methods: Institution based cross-sectional study was conducted from March 25among 413 mothers who delivered with caesarean section. Three governmental hospitals and study participants attended delivery in Addis Ababa. Each study participants were selected byusing consecutive sampling technique was utilized to select individual respondents. The datawas coded, entered and cleaned using EPI Data version 4.6 and then exported into SPSSstatistical software version 26 for analysed. Variables have P-value 0.25 was entered in to multivariable logistic regression model to control for confounder. The degree of associationbetween dependent & independent variables was assessed using odds ratio with 95%confidence interval and statistical significance was declared at p-value 0.05. Results: All the required 413 study participants were interviewed with the response rate of100%. From the total respondents 199(48.2%) of them had good practice of timely initiationof breast feeding. In Multivariable analysis results showed that, there was statisticallysignificance association between timely initiation of breast feeding and parameters such asEducational status of the mother (AOR= 0.18, 95% CI: 0.64-0.50), ante natal care follow up(AOR=0.45, 95% CI: 0.26-0.79), Delivery time (AOR=2.06, 95% CI: 1.28-3.32). Maternalmedical complication (AOR=0.58, 95% CI: 0.35-0.97), mothers feeding for the newbornrather than breast feeding (AOR= 0.22, 95% CI: 0.13-0.38) and maternal breast problem(AOR=9.13, 95% CI: 1.92-43.33). Conclusion and Recommendation: The prevalence timely initiation of breast feedingmothers delivered by caesarean section was 48.2%. Health care providers, especially thoseworking in labour and delivery wards, must give high attention to caesarean section deliverymothers to initiate breast feeding timely after delivery. th -April 30thItem Treatment outcome and Associated Factors of Neonatal Sepsis among Neonates Admitted to Neonatal Intensive Care unit in Public Hospitals, Addis Ababa, Ethiopia, 2021.(Addis Abeba University, 2021-06) Endazanaw Asalf; Mulugeta Tefera; Abebe FikertemariamBackground: Neonatal sepsis is the major cause of neonatal mortality and morbidity globally, particularly in developing countries. Despite studies revealed the extent of neonatal sepsis indeveloping countries, the findings were inconclusive. Identification of the determinants forneonatal sepsis and treatment outcome of newborns with sepsis is not adequate in Ethiopia. Objective: the aim of this study was to assess the treatment outcome of neonatal sepsis and itsidentifying factors among neonates admitted to neonatal Intensive care unit in public hospitals,Addis Ababa, Ethiopia, 2021. Methods- Institution based cross sectional study was conducted among 308 neonates admittedwith neonatal intensive care unit of selected public hospitals in Addis Ababa, Ethiopia. Aftercoding, and entering the data to the software descriptive statistics such as proportion, frequencies, cross-tabulation, and measure of central tendency were calculated. Hospitals andstudy participants were selected by lottery method and systematic random sampling techniquesrespectively. Data were collected using structured pretested questionnaire through face to faceinterview and by reviewing both maternal and newborns profile card. The collected data wasentered by using Epi-data version 4.6 and exported to SPSS version 26 for analysis. Variablesthat have p-value ≤ 0.25 were taken into the multivariable model to control for confounder.Statistical significance was declared at p-value ≤ 0.05. Odds ratio with 95% CI is used todetermine the direction and strength of association between dependent and independent variable. Results: Among the total study 308 neonates, 75(24.4%) were died. Regarding the treatmentoutcome of neonatal sepsis neonates <37 weeks of gestational age (AOR=4.87, 95% CI: 1.2319.22),Grunting (AOR 6.94: 1.48-32.54), Meconium amniotic stained (AOR=3.03, 95% CI:1.02-9.01), Duration of rupture of membrane (PROM) >18hours (AOR=3.66, 95% CI: (1.2011.15),Hypertensive PIH/ Eclampsia (AOR=3.54, 95% CI: 1.24-10.09), Meropenum(AOR=4.16, 95% CI: 1.22-14.21) and CRP positive result (AOR=5.87, 95% CI: 1.53-22.56)were significantly associated with poor treatment outcome of neonatal sepsis. Conclusion and recommendation: The treatment outcomes of neonates 75.6% were recoveredand 24.4% were died. Empirical treatment was the pillar for management of neonatal sepsis inthis setup. There is a need health policy maker to focus on the prevention of risk factors rather than treating the underline disease.
