Patient satisfaction is characterized as a subjective and complex concept, including physical, emotional, mental, social, and social components. Globally, increasing thoughts has been given to the evaluation of patient satisfaction as a strategy of observing for the quality of health care provision in the health care institutions. Quantitative institutional based cross sectional study was conducted and a total of 422 elective surgical patients from 3 public hospitals at Addis Ababa, Ethiopia were included. Study participants were allocated proportionally based on the average surgery done per month in selected hospitals. Data were collected using pre-tested and interviewed structured questionnaires. The collected data was analyzed using SPSS version25.0. Bivariate and multivariate analysis was conducted to examine association between independent and outcome variables. A variable that has P-value of <0.25 bi-variable logistic regression analysis and which can fit binary logistic regression model was subjected for multivariable logistic regression analysis to control the confounding factors. Level of statistical significance was declared at P-value <0.05. The level of satisfaction in this study was mean 83.6 ±0.4. Preoperative fear/anxiety [AOR=0.013; 95% CI: 0.003, 0.067, P= 0.000], previous surgery [AOR= 2.7; 95% CI: 2.51, 6.85, P= 0.000] and Preoperative educations about surgery [AOR=0.005; 95% CI: .000, 0.061, P= 0.000].where the factors associated with level of satisfaction. The level of satisfaction towards preoperative teaching in the current study was 83.6%, more than three fourth of adult elective surgical patients confirmed good satisfaction levels.
