Perceived Enablers and Barriers of Kangaroo Mother Care among Mothers and Nurses at Neonatal Intensive Care Unit of Tikur Anbessa Specialized Hospital, Addis Ababa, Ethiopia, 2020G.C
(Addis Abeba University, 2020-09) Gashaw Mekuriyaw; Murugan Rajalakshimi ; Adimasu Mekonen
Background: Kangaroo mother care is care of preterm and low birth weight infants withearly, continuous and prolonged skin- to-skin contact between the mother and the baby, andwith exclusive breastfeeding. It is recommended for the routine care of newborns weighing2000 g or less at birth. It improves outcomes of premature and low birth weight infants. Eventhough kangaroo mother care is now recognized by global experts as an integral part ofessential newborn care, the adoption and implementation of the kangaroo mother care is stillchallenging. Despite the high impact and apparent feasibility of kangaroo mother care, currently, only a few preterm babies in low-income countries have access to this intervention;coverage of kangaroo mother care has remained low. Objective: To explore perceived enablers and barriers of kangaroo mother care among mothers and nurses in neonatal intensive care unit of Tikur Anbessa Specialized Hospital,Addis Ababa, Ethiopia. Methods: qualitative phenomenological study design was used in the study. A semistructured in-depth interview was applied to explore the perceived enablers and barriers of kangaroo mother care among mothers and nurses in Neonatal Intensive Care Unit of TASH. Atotal of 13 mothers and 7 nurses were included in study with in-depth interview from May2020 - July 2020 and thematic analysis was conducted. Result: The study explored major health staff and setting related, medical condition related, family support related, community support related and caregiver related enablers and barriersof practicing kangaroo mother care among mothers and nurses. Mothers reported that lack ofunderstanding of KMC by their family and the presence of family responsibility and workloadwere barriers to practice KMC. Lack of awareness of KMC by community, social practice and traditional adaptation were also mothers reported barriers. In addition mothers also reported that poor supervision and follow-up, limited resource especially sanitation resource were themajor barriers related to health staff and setting. The presence of back pain and fatigue during kangaroo care also was the barriers reported by mothers. Nurses also reported that scale- up ofkangaroo care was influenced by absence of training, poor attention given by managers andadministrative, shortage of rooms and facilities workload and time shortage related to limitedprofessionals in the ward. Conclusion and recommendation: This study offers how different factors influence KMC utilization among mothers with preterm infants and nurses working at NICU. The finding indicates that a complex array of barriers and enablers determine a mother’s and nurses abilityto provide KMC. To improve mothers’ and nurse’s performance and promote the health ofpreterm infants, supports, such as family, community, and health professional support arerequired. Addressing these factors through policy changes and hospital interventions is essential to enabling optimal neonatal care service.

Recovery Time from Severe Acute Malnutrition and Associated Factors among under-5 Children in Yekatit 12 Hospital, Addis Ababa, Ethiopia.
(Addis Abeba University, 2019-06) Adimasu Mekonen; Sebsbie Girum; Abebe Fkirtemariam
Background: Nearly 20 million under-five children are affected by severe acute malnutrition worldwide. In Africa, more than 14.0 million under-5 children are wasted, of which 4.1million children are severely wasted. Ethiopia is one of the countries with highest under-five child mortality rate, with malnutrition underlying to 28% of all children deaths.According to world health organization and the SPHERE project, at least 75% of childrenadmitted due to SAM should be recovered and they should be recovered in less than or equalto 28 days. Objective: The main objective of this study was to assess recovery time from severe acutemalnutrition and associated factors among under-5 children admitted and treated in Yekatit 12Hospital. Method: A retrospective cohort study was conducted in Yekatit 12 hospital. The studypopulation was under-5 children with severe acute malnutrition who have been managed atYekatit 12 hospital therapeutic feeding unit. The total sample size was 423 and quantitativepretested data abstraction format was used. Kaplan Meier analysis was used to estimate time tonutritional recovery; Log rank was used to test whether the observed difference of recoverytime between different groups of predictor variables is significant or not; and Coxproportional-hazard regression analysis was carried out to determine independent predictors. Result: The nutritional recovery rate was 81.3% and the median recovery time was 15 days(95% CI: 13.608-16.392). By controlling other factors, age, daily weight gain per Kg,vaccination status, comorbidities like pneumonia, stunting, shock, and deworming weresignificant predictors of nutritional recovery time. Conclusion:In conclusion, the overall death rate in this study is poor compared to sphere project referencevalue. However, recovery rate, default rate and nutritional recovery time were in theacceptable range of SHHERE project reference value. Daily Wt gain of ≥8g/Kg, fullvaccination, and deworming with albendazole/mebendazole were proven to reduce nutritionalrecovery time. Conversely, older age, the presence of pneumonia, the presence of stunting, andthe presence of shock as comorbidities were proven to increase nutritional recovery time.

Surgical Patients Satisfaction with Periopeative Nursing Care and its Associated Factors at Public Teaching Hospitals, Addis Ababa,Ethiopia,2021.
(Addis Ababa University, 2021-06) Shamil Fedlu; Tsige Yosef ; Adimasu Mekonen
Patient satisfaction, identified as the achievement or fulfillment of a person's expectations froma service, is one of the most significant metrics for evaluating the quality of health care services in health settings and an indicator of healthcare quality. The purpose of this research was to assess the level of adult surgical patient satisfaction with preoperative nursing care and it sassociated factors at public teaching hospitals, Addis Ababa. Ethiopia, 2021. From February -March 2021, institution based cross-sectional study design with a quantitative approach wasused. A total of 302 surgical patients are selected from public teaching hospitals using random sampling technique and data were collected by face to face interview using structuredquestionnaire. Patient satisfaction was measured using a modified version of the LeidenPerioperative Care Satisfaction Questionnaire (LPPSQ), as well as socio-demographic andclinical data. Data was double-checked for accuracy and quality, then coded and entered intoEPI Info version 7, which was then exported to SPSS version 25.0. To classify variables that were correlated with the dependent variable, bivariate and multivariable logistic regressionmodels were used. P-values of < 0.05 were considered statistically significant analysis. A totalof 302 study subjects were participated making the response rate of 99%. The overall adult surgical patient satisfaction with perioperative nursing care was 65.6%. According to thefindings of this study, age of patients (P value = 0.01, AOR= 8.058), educational status (P value=0.00, AOR= 6.767), type of anesthesia (P value= 0.00, AOR= 4.753), postoperative pain (Pvalue =0.03, AOR= 2.995), and operated agree date and time (P value= 0.00, AOR= 5.370) were all found to be statistically significant predictors of adult surgical patient satisfaction level withperioperative nursing care in multivariate logistic regression analysis. In contrast to othersurveys conducted at home and abroad, the overall level of patient satisfaction with nursing care in this study was good. Patient satisfaction levels should be monitored on a regular basis and allresponsible bodies should work to improve patient satisfaction level.

Survival and Predictors of Asphyxia among Neonates Admitted in Neonatal Intensive Care units of Public Hospitals, Addis Ababa, Ethiopia, 2021.
(Addis Abeba University, 2021-05) Belege Fekadeselassie; Sebsbie Girum; Adimasu Mekonen
Background: Perinatal Asphyxia is an injury that occurs during the perinatal period due tolack of oxygen flow to the fetus or infant, which may lead to ischemia of the brain or otherorgans. Gathering current evidence on recovery time and survival predictors of perinatalasphyxia is crucial to fill scarcity of information. Objective: To assess survival and predictors of asphyxia among neonates admitted inNeonatal Intensive Care Units of public hospitals, Addis Ababa, Ethiopia, 2021. Methods: Hospital-based retrospective cohort study was conducted in four selected public hospitals of Addis Ababa, Ethiopia. Data were collected from February 15 to 15 March, 2021by reviewing medical charts of asphyxiated neonates who were registered from January 1,2016, to December 31, 2020. Epi data version 4.6 was used for data coding, entering and usedSTATA version 16 statistical software for further analysis. Cox-proportional hazardsregression analysis with 95% confidence interval and P-value < 0.05 level of significance wasused to verify predictors of survival of Perinatal Asphyxia. Result: In this study, out of 411(94.5%) participants, 305(74.2%) of them survived during theentire cohort. Overall incidence density rate of survival was found to be 10 (95% CI: 0.080.11)per 100 neonate-days of observation with an overall median recovery time of 8 days.Neonates those who have low birth weight (AHR: 0.67, 95% CI: 0.47- 0.96), stage II HIE(AHR: 0.70, 95% CI: 0.51 - 0.97), stage III HIE (AHR: 0.44, 95% CI: 0.27 - 0.71), occurrenceof seizure (AHR: 0.61, 95% CI: 0.38 - 0.97), thrombocytopenia (AHR: 0.44, 95% CI: 0.24 -0.80) and not supplied calcium gluconate within first days of life after birth (AHR: 0.75, 95%CI: 0.58 - 0.99) were independent predictors of time to recovery of asphyxiated neonates. Conclusion: In this study, recovery time was prolonged in any stages of HIE compared toSaranat’s staging of HIE. This implies early prevention, strict monitoring and takingappropriate measures timely is mandatory before babies transferred into highest stage of HIEfor satisfactory outcomes and reduce mortality.

Time to Death and Predictors among Neonates with Oesophageal Atresia Admitted at Tikur Anbesa Specialized Hospital, Addis Ababa, Ethiopia, 2021.
(Addis Abeba University, 2021-05) Moges Natnael; Sebsbie Girum; Adimasu Mekonen
Background -Esophageal atresia is a developmental defect of the upper gastrointestinal tract in which the continuity between the upper and lower esophagus is lost. In sub-Saharan Africa esophageal atresia has a higher mortality rate ranging from 30- 80%. In Ethiopia, the mortality of neonates with esophageal atresia is reported as 85.3%. To decrease the mortality rate, assessing time to death and predicors of esophageal atresia is crucial. Objective -The objective of this study was to assess time to death and predictors among neonates with esophageal atresia admitted to Tikur Anbesa Specialized Hospital. Methodology -An institutional-based retrospective follow-up study was conducted among 248 neonates diagnosed with esophageal atresia. Data was collected from February 10 to March 10 /2021 by reviewing medical charts of neonates diagnosed with OA who were registered from March 1/2011- February 30/2021 in TASH. A data extraction format was developed by reviewing different literature. Time to death was estimated by using the Kaplan-Meier failure curve and the time to death between different categorical variables was compared using the log rank test. Bivariate and multivariable cox regression hazards models were fitted to identify the predictors of time to death. Hazard ratio with a 95% confidence interval was calculated and p values < 0.05 were considered statistically significant. R Result Among 225(90.7%) study subjects, 71.1% have died. Birth weight esult Among 225(90.7%) study subjects, 71.1% have died. Birth weight. Conclusion -In this study, the proportion of death among neonates diagnosed with esophageal atresia is high as compared to other studies. Among the deaths 76.9 % have died within 14 days of admission. Early diagnosis, appropriate management of comorbidities, and timely surgical intervention will be helpful to overcome the high mortality rate.
