Assessment of Implementation of Quality Management System and Factors Affecting Implementation in Public Hospitals, Amhara Region, Ethiopia

Abstract
Introduction: Total Quality management is one of the most important changes which have
taken place in the field of health care management. The federal ministry of health of Ethiopia
has chosen QMS as a comprehensive and applied strategy to achieve administrative
development and improve the quality of services in order to respond to the needs and
expectations of the society. Since evidences are lacking, there is a needs of break thorough
assessment of existing status of quality management system implementation in hospitals.
Objective: The study aims to assess current implementation status of quality management
system and factors affecting its implementation in public hospital, Amhara region 2017.
Methodology: cross sectional descriptive study were conducted between January 2017-
June 2017, by using both quantitative and qualitative study design to asses’ quality
management system implementation and factors affecting the implementation in randomly
selected seven public hospitals which are found in Amhara region. Implementation status
were measured by 12 standard quality management activities from EHRIG and those who
report the implementation status above mean implementation score were taken as
implemented and those report below the mean as not implemented. The quantitative data
were collected from professionals working in these seven hospitals by using self administered
questioner and those participants were selected by using proportional random sampling and
the qualitative data were collected by using key informant interview from purposely selected
quality committee chairs of the seven hospitals. Data was entered using EPI data software
and analysis was made with SPSS 21 statistical software, descriptive statistics, bivariant and
multivariate logistic regression analysis were applied.
Results: The finding of this study reveal that implementation status of quality management
system of hospitals found in Amhara region was 62% and implementation was found higher
in primary hospitals. six variables were found significantly associated with quality
management system in the hospital.(p <0.05) where Availability of functional quality
committee, Availability of regular and standard based monitoring system of employees QI
activities ,Availability of well communicated quality mission , Providing managers with
training on quality management system with value , Perceived quality significance
employees with p value and Type of hospital with remained to be significantly associated
with quality management system implementation in hospitals

Conclusion and Recommendation: The study concludes that QMS implementation
was found 62.3% beside; its current implementation stage is dependent on the hospital size
where primary hospitals implement QMS more successfully than other hospitals. And
overall, the findings of the study show that there is a need that hospital management should
direct their effort toward communicating the mission and the significance of QMS and,
setting specified budget for QI activities, strengthening employee monitoring system, and
providing support/nurture to quality committees to exceed in QMS implementation
