Background: Esophageal cancer is the eighth most common cancer and the sixth most common cause of cancer relate death worldwide and the fifth most common cancer and attributes a significant number of cancer-related death in Ethiopia. Objective:To asses Computed tomography imaging pattern, risk factors and prevalence of aspiration pneumonia in esophageal ca patients visiting radiology department TikurAnbesa Specialised Hospital for staging. Method: Institutional based prospective cross-sectional study was conducted on 110 histological proven esophageal ca patients come to the radiology department of TikurAnbesa Specialised Hospital for staging.Sociodemographic data were collected by principal investigators using a semistructured questionnaire with the full consent of the participant. The data was collected from the source population and entered and analyzed using SPSS version 25. Text, tables, and graphs were used to present descriptive data, and analysis was processed. Result: Esophagus cancer was commonly seen in the age group between 51 to 60 years (37.3%) with the females (55.5%) more commonly affected than males (44.5%). All patients predominantly presented with dysphagia and weight loss. Dirking or eating extremely hot drinks or meals, alcohol, and smoking was the associated risk factor. The lower esophagus affected more commonly 47.3% compared to other parts. The wall thickness in the majority of the cases measured between 10-20mm (83.8%). T4b N0M0 was the most common staging found in CT (56.7%)with non resectability of 78.18% .Twenty point seven percent of the cases presented with metastasis being lung is the commonest site of metastasis. Squamous cell carcinoma (81.8%) was the most common histopathological type presented. Of 110 patients 12 of them (10.9%) have pneumonia. Conclusion: . Most patients are old age farmers from oromia prsented with 100% dysphagia and weight loss. The identified risk factor are dirking or eating extremely hot drinks or meals, alcohol, and smoking. The lower esophagus affected more commonly 47.3% compared to other parts with significant corraltion with histologic type and T4bN0M0 was the most common staging (56.7%) with 78.18% unresetablility at presentation.en_US
