Background: Approximately 7% of all adult fractures involve the elbow and distal humerus fracture account for 2% of all fracture; with a bimodal distribution affecting young males and elderly females. Nationwide epidemiological data regarding distal humerus fractures are scarce. Surgical management through open reduction and internal fixation (ORIF) is a gold standard and widely practiced, but outcomes remain controversial due to postoperative complications such as stiffness and pain. In this study we will assess the functional outcomes of distal humerus fractures managed with ORIF using the Mayo Elbow Performance Index (MEPI). Objective: To evaluate the functional outcomes and complications distal humerus fractures management with ORIF at Tikur Anbessa Specialized Hospital. Method: A retrospective cross-sectional study was conducted on 53 patients who underwent ORIF for distal humerus fractures between January 2021 and January 2024 at Tikur Anbessa specialized hospital. We collected our data from patient evaluation, medical chart review and assessed the outcome using MEPI score. Results: From the 53 patients, 62.3% achieved good outcomes, 11.3% had excellent results, 13.2% had fair outcomes, and 13.2% had poor results. Common complications included residual pain (65.5%), elbow stiffness (55.2%), and nerve injuries (13.2%). Discussion: Most patients had good to excellent functional outcome following ORIF. However, postoperative pain and stiffness remain prevalent, highlighting the need for early rehabilitation and physiotherapy
