Background: Esophageal Cancer Is a Malignancy of the Esophagus that Arises from Neoplastic Changes in the Epithelial Lining. Computed Tomography (CT) Is Recommended as an Initial Imaging Modality, Primarily to Evaluate Respectability and Detect Distant Metastasis as well us Guides Treatment Options. The Purpose of the Study Is to Assess Diagnostic Accuracy of Preoperative CT Staging of Esophageal Cancer in Patients Undergoing Surgery. Material and Methods: After Ethical Clearance Hospital Based Prospective Cross-Sectional Study were Conducted on CT Scan Reports of 121 Patients who Undergoes Surgery. Diagnostic Accuracy Measured by Comparing with Histopathological Staging. Result: Accuracy, Sensitivity, and Specificity of CT For T3 and T4 Diseases are 50%, 65%, 28% and 67.8 %, 50%, 69.4% Respectively. Diagnostic Accuracy, Sensitivity, And Specificity of CT For the Presence of Nodal Ds Is 62% ,59% And 63% Respectively the Sensitivity of CT For Detecting Tumor-Free Nodes Was High (84.3%) Conclusion: Preoperative CT Has a High Positive Predictive Value for Correctly Identifying Truly Operable Stages. High Sensitivity for Correct Identification of Tumor Free Nodes. Low Diagnostic Accuracy in Correct Prediction of T3 and T2 Stages. Low Diagnostic Accuracy in Identification of Tumor Positive Nodes. Only 23.1.0% Of Patients Had Correct Preoperative T and N Staging Assessment.
