Abstract
Antimicrobials are the most prescribed medications both in hospital and community to relive and treat infectious disease. Antibiotic overuse and incorrect usage are critical factors contributing to antimicrobial resistance (AMR), which is a major worldwide health challenge. Early intervenous (IV) to oral (PO) antibiotic conversion strategy is critical for optimising its utilisation in hospitalised patients. However, the prolonged use of injections when oral formulations would be more suited is a common practice around the world.
