Item Role of MRCP in Biliary Obstruction: Comparison with ERCP/Surgery in Addis Ababa, Ethiopia (2025)(Addis Ababa University, 2025) Habtamu Tilahun; Tesfaye Kebede; Assefa GetachewBackground;-A fast growing noninvasive technique for Imaging assessing the hillary tree and pancreatic ducts is magnetic resonance cholangiopancreatography (MRCP). The effectiveness of MRCP to provide maximum intensity projection(MIP) images that closely resemble those produced by endoscopic retrograde cholagiopancetigraphy (ERCP) without the need for intravenous or oral contrast agents is one of its main advantages . It uses heavily T2 weighted imaging technique or sequences which result in the generation of bright signals from slow moving or static fluids like the bile fluid or secretion from the pancreas making these structures bright and the rest of the background will appear darker.(2) Objectives;-To evaluate the diagnostic performance of MRCP in patients suspected biliary obstruction by assessing imaging features , determining etiologic causes,and comparing its diagnostic accuracy with ERCP and surgical findings Methods;-A total of 112 patients from black lion hospital, lancet general hospital and MC comprehensive specialized hospital were included for this retrospective cross-sectional diagnostic accuracy study . There MRCP images were retrieved from PACs system and then interpreted by the body imaging subspecialist and fellow. Their surgical findings or ERCP findings were retrieved from the operational notes and their medical history were recovered from their hospital medical recording systems . Data was entered to epi info and then analysed with SPSS. Results;-MRCP demonstrated high diagnostic performance, with an overall accuracy of approximately 91.1%, sensitivity of 94.7%, and specificity of 72.2%. The most common cause of biliary obstruction was CBD stones contributing for 43.5 % of cases followed by benign strictures and of no etiology cases .MRCP showed excellent diagnostic performance for CBD stones, with sensitivity exceeding 90% and specificity around 94%. A notable finding in this study was that 16.1% of patients demonstrated biliary dilatation without an identifiable obstructing etiology. Conclusion and recommendation;-MRCP is a highly sensitive and accurate non-invasive imaging modality for evaluating biliary obstruction. It performs exceptionally well in detecting biliary dilatation and CBD stones and is highly reliable in excluding malignancy. However, limitations remain in detecting small stones and subtle lesions and in accurately characterizing stricturesItem Sonographic Patterns of Liver Secondaries among a Patient with Known Primary Extra Hepatic Cancer at Radiology Department of Tikur Anbessa Specialized Hospital, TASH.(Addis Ababa Universty, 2017-10) Abdulhafiz Idris ; Tesfaye KebedeBackground: The most common malignancy of the liver is metastases from other organs. 25–50% patients with a known non-haematological malignancy have liver metastases at the time of diagnosis. The incidence of hepatic metastases depends on the type of primary tumor and its stage at initial Detection. Ultrasonography being used worldwide with high frequency as a reliable method for the initial diagnosis of liver metastases. Aim of study: To assess the common Sonographic patterns of liver metastasis among patient with known extra hepatic primary malignancy. Materials and methods: institution based prospective cross sectional study of 93 patients with known primary extra hepatic malignancy was conducted at sonography unit of Radiology department of Tikur Anbessa specialized Hospital (TASH) Addis Ababa, Ethiopia from February 1 to August, 2017 using a sonoscape SSI-8000 model ultrasound machine. Result: The highest numbers of patients (30.1%) were in the age group of 48 – 57 followed by 38 – 47 and 58 – 67 age group respectively. 81 of 93 patients (87.1%) had multifocal liver metastases disease, where as 12 of 93 cases (12.9%) had single or solitary hepatic metastases. The commonest types of liver lesion identified were target (42%), echogenic (18 %) and Hypo echoic (17 %). Out of 21 primary breast cancer patients 13 patients had Target liver lesion and 5 had hypo echoic liver lesions. Thirteen patients with primary colonic cancer 4 had echogenic, 3 had hypo echoic and 4 had target liver lesions. Conclusion: the most common primary tumor resulting hepatic metastases are those from GI tract with colonic adenocarcinoma being on the top followed by Breast and pancreas. Majority of primary extra hepatic tumor produce multifocal liver metastases .The ultrasound pattern of liver metastases is quite varied and there is no consistent relation between the ultrasound pattern and type of tumor that allows one to specify primary malignant. The various common ultrasound patterns include multiple target, echogenic, hypo echoic and mixed patterns. Other less commonly seen Sonographic pattern of liver metastases is that of complex cystic, calcified and bizarre appearance which we couldn’t easily categorized easily under above common patterns.Item The Effect of Employee Value Proposition (EVP) on Employees' Commitment: the Case of Ethiopian Banking Sectors(Addis Ababa University, 2023-06) Tesfaye Kebede; Adane Atara (PhD)The employment market is getting highly competitive and as the war for talent continues, where employer branding (employee value proposition) can form a competitive advantage, attracting and retaining top talent is not simply a priority but a necessity. This is because, in today’s world, an array of organizations ranging in all sizes, industries, and geographies are facing problems of employee retention, engagement and talent shortage. Accordingly, the purpose of this study was to asses the nature of employee value proposition and examine the effect of employee value proposition on employee commitment in Ethiopian banking sector. In the light of research objectives and questions, only quantitative research approach was adopted and, both descriptive and explanatory research design were utilized. The study was delimited to area sampling and simple random sampling techniques. Quantitative data was collected by using self-administered questionnaire which has been developed and distributed to a sample size of 1947 employees to the selected banks with the response rate of 94.99%.The nature and availability of EVP in the Ethiopian Banking sectors was assesses based on the five parameters of Gartner model and overall it is rated as 58% for Organization Parameter, 56% for Reward Parameters,62%for Opportunity Parameters,62% for Work parameters and 60% for peoples parameters which is moderate need improvement as per the decision rule. Regression analysis was conducted to see the effect of employee value proposition on employee commitment and the findings of the study have shown that employee value proposition attributes Organization parameters show a standardized coefficient value of (0.983) with 0.000significance level, Work parameters show a standardized coefficient value of (0.039) with 0.000 significance level, Opportunity parameters show a standardized coefficient value of (-0.027) with 0.000 significance level and People parameters show a standardized coefficient value of (-0.017) with 0.001 significance level and Reward parameters show a standardized coefficient value of (0.006) with 0.162 significance level and have no significant effect on employees’ commitment. Based on the result of the study, it is recommended that to increase the commitment level of their esteemed employee and to enhance the attraction practice of talented employee from the labour market, motivate the existing employee and retain their top performers, in return to increase organizational performance, the Ethiopian banks should have to work on developing a compelling employee value propositionItem Urinary Bladder Cancer :Value of Ct and Mri in Differntiating Non Muscle Invasive Blader Cancer From Muscle Invasive Bladder Cancer with Histopathologically Staged Tumors(Addis Ababa University, 2024-12-19) Mahlet Kifle; Tesfaye KebedeBladder cancer ranks as the tenth most prevalent cancer globally, representing 2% of all malignant urinary tract tumors. Diagnosis entails utilizing Cystoscopy first, followed by TURBT and histology. Imaging is primarily valuable for determining the stage. CT imaging is useful for assessing larger, higher-staged tumors and for metastatic evaluation. Meanwhile, MRI has the potential to provide value in evaluating local and regional staging, as well as assessing muscular invasion in bladder cancer, which is crucial for prognosis and treatment decisions.
