Item Adolescent Communication to their Parents on Sexual and Reproductive issues and Associated Factors Among Secondary School Students in Meki Town, East Shoa, Oromia, Ethiopia: Mixed Method Study 2023(Addis Ababa Unaiversity, 2023) Furo Barke; Berhe Semarya; Genzeb SirayeBACKGROUND: Adolescents are more vulnerable than any other age group to a range of sexual and reproductive health problems. Adolescents are exposed to various sexual and reproductive risks due to lack of communication with their parents, such as early-unprotected sex, early pregnancy, school dropout, sexually transmitted Infections (STIs) and human immune virus (HIV). OBJECTIVES: To assess adolescent parent sexual and reproductive health communication and associated factors among secondary school students in Meki town, East Shoa, Oromia, Ethiopia 2023. METHODS: Institution based convergent mixed method was applied. The study was conducted from February 20- March 20/2023 among 392 students at Meki town. The quantitative data was collected using pretested structured self-administered questionnaire by systematic sampling technique. Data was entered using EPI data version 4.6.0.6 and then transport to SPSS window version 25 for statistical analysis. The association was determined using 95% CI and p value (<0.05) for multivariable logistic regression. The qualitative data was collected using interview guides(open-ended). The response was tape-recorded. In-depth interview was conducted among purposively selected parents. Thematic analysis was used using Atlas ti 9 software. Result: The proportion of respondents who had communicated with their parents regarding SRH issues on at least two components was 156 (39.8%). Being grade 10 student (AOR = 0.44: 95% CI; 0.225-0.862), merchant mothers (AOR =2.76: 95% CI; 1.36-5.62), attitude toward SRH (AOR= 0.182 95% CI, 0.1-0.333), adolescent had SRH information (AOR=3.28; 95% CI 1.3-8.23) and knowledge of SRH issues (AOR=1.75: 95% CI: 1.03-2.98) were significantly associated. Five themes were discussing about SRH components, Source of SRH information,approach to Improve SRH discussion, SRH reason and challenges for not discussing and SRH discussion age and satisfaction. Conclusion and Recommendation: There was insufficient adolescent-parent communication regarding SRH components. The main reason from both quantitative and qualitative study was cultural barriers, fear of parent response, lack of trust in adolescent, morally unacceptable and shame to talk. The Ministry of Health has to give focus and facilitate health care professional, different government sector, and nongovernmental organization to work on it.Item Assessment of the Practice of Obstetric Care Providers in Initial Management of Post-Partum Hemorrhage and its Associated Factors, at Selected Health Centers of Addis Ababa, Ethiopia, 2021(Addis Ababa University, 2021-05) Shewanawul Etabezahu; Berhe Semarya ; Assefa YeshiBackground: Post-partum hemorrhage (PPH) is one of the foremost reasons of maternal death globally. in Ethiopia 80% of maternal death is associated with PPH, there is no enough paper to assess the practice of mid-level health professionals specially in initial PPH management as primary respondents. Objective: to assess the practice of obstetric care providers in initial management of PPH and its associated factors at selected health centers of, Addis Ababa, Ethiopia, 2021. Methods: A descriptive cross-sectional facility-based study design was conducted with sample of 561 respondents. Through logistic regression in the bivariate analysis variables with p value at 0.25 was candidate for multivariate. And multivariate analysis was used to determine the association between the outcome variable. 95% CI used for association and pvalue <0.05stated as statistically significant. The result was explained in text,chart,graph and tables. Result:In this study finding 53% of the study participants had good initial post-partum hemorrhage care practice, while 47% had poor practice. according to multivariable regression,male participants had 1.5 times good practice on PPH initial management than female (AOR=1.5, 95%CI=1.97, 2.18). the odds of being midwife in profession had almost two times good practice on PPH initial management than health officer (AOR=1.9, 95%CI=1.92, 3.70).Participant who has 4 year and above experience in labour ward has 2.5 times good practice onPPH initial management than duration of work less than six months in labour ward (AOR=2.5,95%CI=1.09, 5.87). Participant who had training in BEMNOC had 3 times good practice onPPH initial management than had not training on BEMNOC (AOR=3.3, 95%CI=1.91, 5.76). Conclusion and recommendation: in this study the overall practice for initial management of postpartum hemorrhage by mid-level health professional in health center level were (53%)good practice. the remaining (47%), poor practice could fulfill by training of professionals with updated PPH management, follow protocol and improve education. the determinant variable that affects positively were experience in labor ward, midwife profession, sex of participants and training in obstetric based management systems.Item Demand for Long Acting and Permanent Contraceptive Methods and Associated Factors among Married Women Visiting Assosa Governmental Health Institution for Family Planning, 2019(Mixed study).(Addis Ababa University, 2019-06) Birku Zelalem; Berhe Semarya; Assefa YeshiBackground: - Demand for long acting and permanent contraceptive method is one of the key factors that determine the fertility and the reproductive health of the women. These demand for x LAPM was not consistent with and the utilization of LAPM. Objective: - to assess demand for LAPM and associated factors among married women visiting Assosa governmental health institution for family planning service. Method: - institutional based cross sectional study were conducted to assess demand for LAPM and its associated factor from April 12-May 10/2019 for total of 251 sample size. For the qualitative study, in-depth interview was conducted on purposively selected key informant. The collected data were entered into Epi info- 7 and the data were cleaned and analyzed by SPSS 24. Bivariate and multivariable analysis was used to identify the association between dependent and independent variable. The variable having pvalue=<0.2 were entered into multivariate regression and selected variable in multivariate at p<0.05 weredeclared as having significant association and AOR with 95% CI was used to measure the degree of association. RESULT:-the demand for LAPM at the study area was 158(62.9%).Awareness about LAPM (AOR=2.50395%CI=1.188-5.2274),knowledge of LAPM (AOR=2.62, 95%CI =1.295-5.299),previous utilization of LAPM (AOR=3.369, 95%CI=1.805-5.291)and couple discussion about any LAPM(AOR=2.067,95%CI=1.027-4.163)were associated with demand for LAPM. Fear of theside effect was the main reason for non utilization of LAPM. Conclusion:-demand for LAPM was not consistent with the utilization.The finding shows that,62.9% of the respondent had demand for LAPM. Recommendation:Ineeds to do more on the limiting factors to utilize LAPM.so MOH, RHB, theinstitution manager together with health care provide and different NGO work more to meet the increasing demand for LAPM.Item Discontinuation Rate of Long Acting Reversible Contraceptives and Factors Associated among Reproductive Age Women in Butajira Town, Southern Ethiopia, 2020.(Addis Ababa University, 2020-06) Asnake Haregwa; Berhe Semarya; Sneshaw WorkneshBackground: High prevalence of modern contraceptive discontinuation including long-acting reversible contraceptives without any change in fertility intention has resulted in unintended pregnancy. Discontinuation of contraceptives often associated with unplanned pregnancy whichends up with induced abortion and unplanned birth. The DHS analysis of 60 countries reported that discontinuation rate of an IUD was 14.3%. Based on the report of developing countries 13.1%, 26.3% and 36.7% of IUD user women discontinued at the first 12, 24 and 36 months respectively. Objectives: To assess the discontinuation rate of long-acting reversible contraceptives and factors associated among reproductive age women in Butajira town, Southern Ethiopia, 2020. Methods: community-based cross-sectional study was conducted from April to May to assess the discontinuation rate of long acting reversible contraceptives and factors associated among reproductive-age women in Butajira town. Systematic sampling method was used to select 227women. Data was collected by questionnaire and entered into a computer using Epi-data version 4.6.2 statistical program then it was exported to SPSS version 25 for analysis. Statistical analysis was used to predict the relation between dependent and independent variables. Lastly, significant of statistical association was assured using OR at a 95% confidence interval and p-value <0.05.The result was presented using text, tables, graphs and charts. Result: this study revealed that discontinuation rate of long-acting reversible contraceptives was found 22.5%. The main reason for discontinuation was facing of side effect 26(52%) and from that menstrual disruption 19(73.1%). Time consumed to reach the health facility (AOR=6.16(95% CI 1.46-25.97), desire to have more children (AOR= 5.3(95% CI: 1.13-24.81) and counseled about benefit of long acting reversible contraceptives (AOR=0.08(95% CI: 0.02-0.39) were predictors of LARC discontinuation. Conclusion and Recommendation: This study showed that discontinuation rate was 22.5%.The result suggested that need of implementing and strengthening regular supervision on how clients are counseled.Item Efficacy of Transcervical Foley Catheter Versus Intravaginal Misoprostol as a Cervical Ripening Method and Associated Factors in Selected Public Hospitals of Addis Ababa, Ethiopia, 2020.(Addis Ababa University, 2021-01) Marye Shimels; Berhe Semarya ; Assefa YeshiBackground: Many women who undergo labor induction have an unfavorable cervix. Therefore,cervical preparation by using a safe and effective method is necessary to increase the chance of successful induction. Objective: To compare the efficacy of transcervical Foley catheter and intra-vaginal misoprostol as a pre-induction cervical ripening method and associated factors at selected public hospitals of Addis Ababa, Ethiopia, 2020. Methods: A retrospective institution based cross-sectional study was conducted from October to November 2020 in selected Hospitals of Addis Ababa, Ethiopia. The consecutive sampling method was used to enroll a sample size of204 study participants for each group of cervical ripening methods, by using delivery and induction registration books. The first group was women primed by transcervical Foley catheter and the second was those by intra-vaginal misoprostol. Data were collected by questionnaire and entered into a computer using Epi-data version 4.6.2 statistical program then it was exported to SPSS version 25 for analysis. Selected baseline characteristics and outcome measures were compared using the independent sample T-test and Fisher’s exact or Chi-Square test for continuous and categorical variables respectively. Lastly, the significance of the statisticalassociation was assured using OR at a 95% confidence interval and p-value <0.05. Result: The rate of success of induction (χ2 = 5.892; P=0.015) and mean change in Bishop's (P<0.001) score were found to be significantly higher in the Foley catheter group. But the rate of uterine hyperstimulation (P =0.02), fetal heart rate abnormality (P=0.01), maternal and neonatal adverseeffects (P=0.04), and NICU admission (P=0.01) were significantly higher in the misoprostol group. Parity (AOR=4.45(95% CI: 2.01- 9.82) and gestational age (AOR=0.45(95%CI: 0.220.90),were significant predictors of success in misoprostol group compared to residential address (AOR=0.46(95%CI: 0.26-0.95) in the Foley group. Conclusion and recommendation:This study showed that a transcervical Foley catheter increases the success rate of induction and decreases maternal and neonatal complications associated with induction of labor. The result suggested that safety issues need to be taken into consideration when misoprostol is planned to use especially in multiparous women.Item Incidence of Surgical site Infection and Factors Associated among Cesarean Deliveries in Selected Government Hospitals in Addis Ababa, Ethiopia, 2019 G.C.(Addis Abeba University, 2019-06) Lijaemiro Hana; Berhe Semarya; Tesfaye JembereBackground: One to two third of operated patients in low-income countries acquire surgical siteinfections, which is nine times higher when it is compared to high – resource countries.Identifying the incidence and risk factors that contribute for surgical site infection followingcesarean delivery is a step ahead for preventing and reducing the problem. Nonetheless, the distribution of the problem in Addis Ababa, where the rate of cesarean delivery is relatively high compared to other parts of the country, is under investigated. Objective: The aim of this study is to assess the incidence of surgical site infection among CDs and factors associated with it in selected government hospitals in Addis Ababa, Ethiopia, in 2019. Method: A hospital-based prospective cohort study design was employed to follow 175 women, who gave birth by CD inselected government hospitals in Addis Ababa, from March 11 – April 19, 2019. Convenience sampling method was used to select study units from the randomly selected hospitals.Descriptive statistics were run for determining rate of CD SSI. Presence and degree of association between outcome and independent variables was computed through bivariate logistic regression analysis. Factors that had ≤ 0.2 significance level in the bivariate logistic regressionanalysis was considered in the multivariable logistic regression analysis. AResult: From 166participants who completed 30 day follow-up, 25 (15%) of the participants developed SSI. Age,gestational age, and duration of operation showed a significant association with the outcomevariable with AOR (95%CI) of [(AOR = 1.504, 95% CI: (1.170 – 1.933))], [(AOR = 0.019, 95%CI: (0.001 – 0.291))], and [(AOR = 1.108, 95% CI: (1.025 – 1.197))] respectively. Conclusionand recommendation: SSI rate is higher in this study. Certain associations may be lost due tosmall sample size. Further interventional studies with vast sample size are recommended.Enhanced regulation of infection control policy, implementation of improved surgicaltechniques, and persistent hand hygiene among respective stakeholders can avert the condition.Item Knowledge and Attitude towards Human Papillomavirus Vaccine and Associated Factors among Mothers Who have Eligible Daughters in Debre Markos Town, Northwest Ethiopia, 2021(Addis Ababa University, 2021-06) Tesfaye Melkam; Berhe Semarya; Genzeb SirayeBackground: Human papillomaviruses are a big group of highly ubiquitous, small viruses. It is a sexually transmitted disease. Both women and men are rapidly exposed to it after the onset of sexual intercourse. Even though, parents have an important role in the decision-making process regarding the HPV vaccination for their child. Less emphasis is given to their knowledge and attitude towards HPV vaccine; mainly to mothers. Objective: To assess the level of knowledge and attitude towards human papillomavirus vaccine and associated factors among mothers who have eligible daughters in Debre Markos town, Northwest Ethiopia, 2021 Methods: A community-based cross-sectional study was conducted among 601 mothers who have eligible daughters and selected by consecutive sampling technique from February 16/2021 to March 16/2021. The data was collected by using face-to-face interview administration and entered into EPI data version 3.1 then, exported to SPSS version 25 for analysis. Variables with a p-value of < 0.25 in bivariate were a candidate for multivariate andthose variables with a p-value of <0.05 in multivariate logistic regression were considered as a statistically significant factor for knowledge and attitude of HPV vaccine. Result: A total of 601 study participants with a response rate of 100% were included in the study. Among participants in this study, 47.6% have good knowledge and 77.4% have positive attitude towards HPV vaccine. Knowledge of mothers about HPV vaccine was affected by having degree or higher (AOR=7.687; 95 % CI=1.837-32.168; p=0.005), not hearing about HPV vaccine (AOR=0.172; 95%CI=0.098-0.302; P=0.000), and positive attitude towards HPV vaccine (AOR=2.959; 95%CI=1.580-5.539; P=0.001). Attitude of mothers towards HPV vaccine was affected by not hearing about HPV vaccine (AOR=0.285; 95%CI=0.163-0.499; P=000), and good knowledge about HPV vaccine (AOR=2.705; 95 % CI=1.454-5.035; 0.002). Conclusion: knowledge about HPV vaccine was low. Maternal educational level, hearing about HPV vaccine and attitude towards HPV vaccine were factors associated with knowledge towards HPV vaccine. However, attitude was high and factors associated with attitudes towards HPV vaccine were knowledge about HPV vaccine and hearing about HPV vaccine.Item Knowledge, Attitude, and Practice of Medication Abortion and Associated Factors Among Reproductive Age Women in Selected Sexual and Reproductive Health Clinics of Addis Ababa, Ethiopia, 2018.(Addis Ababa University, 2018-06) Yeshambel Addisu; Berhe SemaryaBackground: Abortion rates following unintended pregnancies is increasing in developing countries like Africa and unsafe abortion is commonly neglected reproductive health care problem. In Ethiopia, the number of women receiving treatment for complications from unsafe abortion nearly doubled from 2008-2014 which is 52,600 to 103,600. Medication abortion is one of the safest abortion interventions. Adequate level of knowledge and attitude among women of reproductive age group on MA contributes to prevention and control of unsafe abortion and the resulting mortality and morbidity. Objective: Knowledge, attitude and practice of medication abortion and associated factors among reproductive age women in selected SRH clinics of Addis Ababa, Ethiopia, 2018. Method: A cross-sectional quantitative study design was conducted to collect data from study participants in SRH clinics of Addis Ababa from February–March 30, 2018. Study participants were selected through systematic random sampling based on their proportional distribution of sample size to each clinic. Structured questionnaire was used to collect data. The data was coded and entered into Epi data version 4.2 and the analysis was carried out by using SPSS version 23. Bivariate and multivariate analysis with 95 % CI was employed. Variables found to have a P-value<0.2 in the binary logistic regression were entered into multivariate analysis and strength of association was declared at P value<0.05. Results: From the total of 423 mothers interviewed 412 responded the questionnaire with a response rate of (97.4%).The overall knowledge, attitude and practice of MA was 72.1 %, 44.2% and 33% respectively. Majority (36.5%) of the study participants would advise someone with unwanted pregnancy to undergo an abortion and 55.6% would consider abortion if they had unplanned pregnancy. From 205 respondents who had ever terminated their pregnancy, 136 (66.7%) used MA. Participants level of education, father education; income and history of unwanted pregnancy were some of the factors associated with MA. Conclusion and recommendations: this study provided that women were relatively knowledgeable but their attitude and practice on MA is low. Low attitude and practice of MA calls health care providers giving due attention on MA, women empowerment, awareness creation on MA in the community, strengthened action to provide quality of maternal care for all childbearing mothers towards MA and unsafe abortion in particular.Item Prevalence of Fatigue and Associated Factors Among Adult Cancer Patients Attending Tikur Anbessa Specialized Hospital, Addis Ababa, Ethiopia, 2019.(Addis Ababa University, 2019-06) G/her Teka; Berhe Semarya; Tesfaye JembereBackground: Fatigue is a common distressing problem in most cancer patients. It may be caused by the disease, the complications of the disease and its treatments. This study aimed to assess prevalence of fatigue and associated factors among adult cancer patients receiving various treatment. A cross sectional study was conducted on 278 patients from February 2019 to march 2019 in TASH Cancer Center. Brief Fatigue Inventory (BFI) scale was used for assessment of fatigue, other questions were concerning the socio-demographic data, medical condition and clinical factor included. Objectives: was to assess the prevalence of fatigue and associated factors among cancer patients February and March 2019, at TASH in Addis Ababa, Ethiopia. Methods: A cross-sectional study design was used among 278 randomly selected cancer patients undergoing cancer treatment at TASH. A standardized data collection tool, Brief Fatigue Inventory (BFI) was used to gather information from participants. The data was entered to EPI-data version 3.1 and transferred to SPSS version 24 for analysis. Bivariate and multivariable logistic regression were conducted to summarize the data. Level of confidence was determined at 95% CI and p-value <0.05 was considered significant for associations. Result: The prevalence of fatigue among cancer patients in this study showed (74.8%). Age AOR=3.15[1.352-7.336], stage of cancer, AOR: 0.022, [0.0030-0.172], presence of infection, AOR: 4.153 [1.064-16.069], type of cancer AOR= 5.189 [1.590-16.9], and type of treatment AOR: 0.177 [0.068-0.46] showed significant association with fatigue. Conclusion and Recommendation: The prevalence of fatigue among cancer patients undertaking cancer treatment in this study is high. Patients undergoing radiotherapy and chemotherapy demonstrated higher fatigue symptoms than the other treatment modalities. Age, stage of cancer, type of cancer, type of treatment and infection were associated with fatigue. Therefore, especially nurses and physician could be work on how to reduce fatigue at all levels of patient care. The total cost of the study was 25000.09 ETH Birr.
